Laboratory Designation Application

The state of Ohio now accepts applications from laboratories that wish to become approved as a designated testing laboratory.  One criterion is that the laboratory must have the ability to test blended fiber products and report the findings to our office.  The laboratories are located both in the United States and internationally.  Blended fibers are typically found in mattresses, but also appear in filling materials for pillow shams and quilts and a few other products.   If you’re a manufacturer of products that contain blended fibers, you’ll want to contact an approved designated laboratory to have them tested. If you are a lab that would like to become an approved testing laboratory by the Ohio Department of Commerce, you can submit an application and supporting documentation that confirms your credentials and testing qualifications through the form on the right. Approved laboratory designation is valid for 2 years from the date of approval and the designee must reapply at least 30 days prior to the expiration date.

How to Become an Approved Designated Testing Laboratory

•    Complete the Ohio Department of Commerce application titled “Designated Laboratory Application.”

•    Applicant must provide the following information and documentation when submitting the application:

o    Complete name of laboratory along with complete address and contact information.  
o    Accreditations and credentials held by applicant.
o    Name and title of the person submitting the application

•    Applicant must provide a description of the test methods utilized that determine the filling content and their percentages in regulated articles of bedding, upholstered furniture, and stuffed toys.
•    Applicant must provide a sample document of the analysis report that clearly reflects the test method(s) utilized and the description of the contents determined. This document will need to identify the name, address, and contact information of the manufacturer submitting item(s) tested. The report will need to indicate the description and date the sample was tested. A photo of the sample tested will need to be provided with the report.  

•    The approval designation applies to all satellite laboratories owned and operated by the applicant.
