$65 per boiler. If payment is not made within 45 days a $12.50 late fee will be added per boiler. Make check or money order payable to: Treasurer, State of Ohio.

4.What is a Certificate of Registration?

This certificate is issued to contractors installing and making welded repairs to boilers.

5.How do I become registered with the Bureau to install or make welded repairs to boilers?

You will need to complete the Boiler Contractor Application.

6.What is the fee for the Certificate of Registration?

There is an annual fee of $50.00. Make check or money order payable to: Treasurer, State of Ohio.

This permit is issued to a registered contractor that would be installing or making welded repairs to a specific boiler.

You need to complete the Boiler Installation/Repair Permit Application.
