Public Safety Vehicle Forms & Information

The volunteer fire, rescue, and emergency services in Ohio can use personal vehicles as public safety vehicles through the Ohio Revised Code. These vehicles must be properly identified, be responding to fire, emergency or EMS calls, and the person operating the vehicle must be a qualified member of a volunteer fire, rescue, and/or EMS agency.

Below are the instructions and forms required to use a personally owned vehicle as a public safety vehicle. This must be renewed on an annual basis.

Filing instructions for public safety vehicle decals

  1. Vehicles shall be inspected by the Ohio State Highway Patrol (OSP) and pass a safety inspection.
  2. The fire department shall retain a copy of the OSP vehicle inspection.
  3. Columns A through F must be completed for all vehicles receiving a date sticker.
  4. Indicate if a Maltese Cross is needed with an (X) in column G. A Maltese Cross should only be requested for new members, new vehicles or to replace a damaged decal.
  5. When a vehicle is sold, wrecked, or a member leaves the department, have the vehicle removed from the approved by list by filling in columns B, C, D, E,and I. Additionally, the decals must be removed from the windshield.
  6. The Maltese Cross and date sticker must be placed in the lower right-hand corner of windshield.
  7. This form should be received by the State Fire Marshal’s Fire Prevention Bureau no later than 90 days after the date of inspection.
  8. Fire departments must be current on submittal of fire incident reports beforethe decals are issued.
