The Video Service Authorization Program regulates cable television providers. The VSA program does not regulate satellite companies, Internet or phone service provided by cable television companies.

When completing the form below, be specific, explain the nature of your complaint; include names of persons you’ve talked to at the cable provider and when, etc. Feel free to attach documents to support your complaint.

In addition to completing the form below, you can also call the Cable Complaint Hotline directly at (877) 207-2225 or mail in your complaint using the printed version of this form.

When you submit a complaint you are certifying that all of the information provided is true and that a copy of your complaint, along with supporting documents, will be sent to the cable company. The VSA program serves as a facilitator to try to resolve disputes between a subscriber and the company and cannot represent you in legal proceedings.
